Primary Health Care
Our Primary Care team provides regular ongoing health care which includes health promotion, disease prevention, and health maintenance. Our clients are considered a partner in their own health and play an important role in the care plan, which enables them to be the best that he or she can be. We monitor chronic illness, as well as see people on an urgent basis for episodic illness. Referrals are made to specialists by the physicians and the nurse practitioners if required.

- Diabetes Prevention: Family Kitchen, Garden Photo Challenge
- Smoke Free Ontario: Smoking Cessation Support Groups/Programming
- Jays Care-Indigenous Rookie Baseball League: 7 Community Teams, 21 regular season games, 16 end of season tournament games, a total of 149 participants, and 28 coaches.
