The Diabetes Program is divided into two components. Both components are funded by MOHLTC and NELHINS.

Minomodzawin Diabetes Program

Minomodzawin means “To Live Well”. Limited resources are flowed to each of the First Nations to provide health promotion and activities for THE PREVENTION OF DIABETES. This program is supported by the Diabetes Nurse Educator, Registered Dietician, Community CHN and/or Community Diabetes Lay Educator.

What We Do

  • Provide group and/or counselling and education sessions
  • Provide nutritional counselling
  • Conduct individual assessments and provide follow-up support
  • Give clients the tool to self-evaluate behaviours and outcome

Naadmaadsaan Diabetes Team

Naadmaadsaan means “Helping One’s Self”. This program is supported by the Diabetes Nurse Educator, Registered Dietician, and the services of a Foot care specialist. This program will have you engaged in a healthy lifestyle to manage your diabetes so that your quality of life can be maintained or improved.

What We Do

  • Provide comprehensive education in Diabetes Self-Management
  • Provide group and/or individual counselling and education sessions
  • Provide nutrition counselling
  • Conduct individual assessments and provide follow-up support
  • Provide Referrals to foot care services
  • Collaborate with clinic outreach services for high risk populations
    (Gestational Diabetes and Children with Diabetes)
  • Give clients the tool to self-evaluate behaviours and outcome
  • Provide referrals to other health professionals as required
