Preamble
We are committed to protecting the personal health information that we collect from you. Please use this as a guide to understand how we collect, use and disclose the information that you provide to us.
N’Mninoeyaa Aboriginal Health Access Centre and Baawaating Family Health Clinic are committed to protecting every clients’ health information in order to continue providing safe and quality health care. Within our health care model, we operate through a Circle of Care. The Circle of Care is to ensure that you receive the most appropriate care with the right provider at the right time.
Purpose
The purpose of the Circle of Care is to work with you to identify which health care providers will have access to your health information in order to support your needs.
You will be required to sign a consent form that allows for your health information to be shared for the following purposes:
- Provide you with health care services within our organization
- Plan or deliver health programs or services
- Arrange internal and external referrals
- Complete administrative duties (i.e., program reports and performance monitoring)
- Gather statistics for programming, monitoring and/or research (your specific health information is not used, aggregate data only)
- Teach healthcare professional students who are with us during their clinical placements
- Fulfill other purposes permitted or required by law (i.e., Duty or Report, Reportable Diseases and Subpoenas)
If the organization needs to disclose and/or use your personal health information beyond the above criteria, we are required to ask you for additional consent.
Circle Of Care
The N’Mninoeyaa Aboriginal Health Access Centre and the Baawaating Family Health Clinic operate and provide services to you under the principle of the Circle of Care. The Circle of Care is a team of health care providers that will need access to your health information in order to work closely with you to make decisions regarding your health care plan.
Your Circle of Care may include, but is not limited to:
- Doctors/Physicians
- Nurse Practitioners
Client Care Coordinators - Community Health Nurses
- Registered Dietitians
- Diabetes Nurse Educators
- Physiotherapists
- Occupational Therapists
- OT/PT Assistants
- Hospital Discharge Nurses/System Navigators
- Traditional Health Providers
- Mental Health and Addiction Workers
- Social Workers
- Pharmacists
Limiting Collection
It is always your choice to provide us with your personal health information. You may withdraw your consent at any time (subject to legal restrictions and with reasonable notice). You may limit the information that we are able to share, or limit who we share it with. Any limitation put on your personal health information may restrict the services we are able to provide to you. We will do our best to resolve any concerns that you may have so that we can work together to provide you with quality health care.
Limited Use, Disclosure & Retention
We will only use or share the information you give us to reach your health care goals. We are legally required to keep your information for ten (10) years, and for children, ten (10) years after their eighteenth (18th) birthday (Regulated Health Professions Act). Accuracy
We will keep your personal health information as accurate, complete and up-to-date as possible.
Safeguards
Our files, computers and any location where health information is stored is kept secure. We ensure that all staff and students have signed a confidentiality agreement and have received training on the organization’s privacy policies. N’Mninoeyaa, Baawaating and all partnering health centres adhere to the government regulations set out in the following Acts:
- Personal Information Protection and Electronic Documents Act (PIPEDA)
- Personal Health Information Protection Act (PHIPA)
In the unlikely event that your personal health information is stolen, lost or accessed by unauthorized persons, you will be notified immediately.
As a health services organization, we review our privacy policies and procedures annually to ensure all privacy, security and confidentiality practices are adhered to. We also closely monitor our electronic medical records (EMR) by running regular audits to ensure your health information is safe.
